ADDITIONAL INFORMATION:
- Additional time and roller skating instructor available upon request. (Additional charges) Please send your request by email to rollerdiscomontreal@gmail.com
- If your group has participants between the ages of 5 and 15, please email Roller Disco Montreal to request more details. Please include your name, email, phone number, date and time of the event, age of the group, and number of participants.
- 7+ days prior to the event, please inform Roller Disco Montreal of the skate sizes (female - male) required for the event.
- The Total Amount, non-refundable, is required at the time of booking. Electronic transfers are accepted via rollerdiscomontreal@gmail.com
- No refund of amounts received in case of cancellation by the customer.
- Roller Disco Montreal reserves the right to cancel the event due to unforeseen circumstances beyond their control.
- If the event is cancelled by Roller Disco Montreal, the client will receive a full refund of any amount paid for services not rendered.

The form (completed in full) must be brought on the day of the event and given to the Roller Disco Montreal staff. If the person is under 18 years of age, please print their NAME (NAME field) and the parent-adult must sign (SIGNATURE field) in their name.
